Responsibilities:
– Provide nursing care and support to patients in their homes or community settings.
– Conduct assessments, administer medications, and dress wounds as necessary.
– Educate patients and their families on managing their health conditions and accessing resources.
– Work collaboratively with other healthcare professionals to coordinate holistic care.
Requirements:
– Registered Nurse qualification with active NMC registration.
– Previous experience in community nursing or a similar role.
– Strong clinical skills and the ability to work autonomously.
– Valid driver’s license and access to a vehicle for home visits.
